- I, the undersigned, hereby acknowledge that I am a member of Kenya Management Staff Union (KMSU).
- In accordance with the Labor Relations Act 2007, the Constitution of Kenya 2010, the Employment Act 2007 and the Labour Institutions Act, I authorize the deduction of 2% of my gross pay from my salary as my trade union monthly subscription dues, being the employee who has acknowledged membership of this Union.
- These remittances, along with the corresponding notices, should be made not later than the 10th day after the month in which the deductions were made.
- Registration fee is Ksh 1,000, and shall be payable upon submission of this form to the Kenya Management Staff Union bank account.
- All financial payments shall be made to the Kenya Management Staff Union (KMSU) Bank Account no.1322391327, Kenya Commercial Bank.
- The employer to whom this notice is addressed is required to make returns to the Registrar of Trade Unions, as stipulated by the order of the Cabinet Minister.

- This notice applies to wages payable by the employer to you at the end of the month following the date indicated herein.
